Help Build a Veteran-Focused Rehabilitation Program
Level Up Medical is seeking an Arizona-licensed Physical Therapist to help launch and develop our physical therapy service line serving Veterans in Southern Arizona on Tucson's' Eastside.
Level Up Medical is an established provider of physician-referred medical massage through the VA Community Care Network. We are expanding into integrated physical rehabilitation through a collaborative model incorporating physical therapy, rehabilitative exercise, medical massage, patient education and whole-person wellness.

- Conduct comprehensive physical therapy evaluations and re-evaluations.
- Establish individualized, evidence-informed plans of care.
- Determine appropriate treatment goals, frequency, duration and interventions.
- Delegate appropriate treatment activities to the PTA.
- Provide PTA supervision consistent with Arizona law, payer requirements and professional standards.
- Review patient progress and modify the plan of care when clinically necessary.
- Remain available for clinical consultation when a patient’s condition or treatment plan changes.
- Complete accurate and timely documentation.
- Ensure treatment remains within the scope of each VA Community Care authorization.
- Communicate with referring providers when medically necessary.
- Collaborate with Level Up’s PTA, medical-massage providers and administrative team.
- Help establish physical therapy workflows, documentation standards and clinical policies.
- Participate in outcomes measurement and quality-improvement activities.
- The PTA will provide much of the ongoing treatment and routine patient coordination under the DPT’s established plan of care.
- The DPT will retain responsibility for clinical decision-making, plan-of-care oversight and PTA supervision.

Patient Population
The physical therapy program will primarily serve:
- Veterans referred through the VA Community Care Network
- Individuals with chronic musculoskeletal pain
- Patients recovering from orthopedic injuries and conditions
- Patients experiencing mobility, balance or functional limitations
- Active adults working to regain strength and physical independence
- Required Qualifications
- Graduate of a CAPTE-accredited physical therapy program or a program holding CAPTE Candidate for Accreditation status
- Current, unrestricted Arizona Physical Therapist license or eligibility to obtain Arizona licensure
- Current individual Type 1 NPI or ability to obtain one
- Professional-liability insurance
- Current CPR/BLS certification
- Ability to complete AZ Blue and VA Community Care credentialing
- Complete CAQH ProView profile or willingness to establish one
- Strong clinical documentation and patient-education skills
- Ability to supervise and collaborate effectively with a PTA
- Commitment to evidence-informed, patient-centered care
Preferred Qualifications
- Doctor of Physical Therapy degree
- Outpatient orthopedic or musculoskeletal experience
- Experience treating Veterans, service members or first responders
- Experience supervising a PTA
- Manual therapy or movement-assessment training
- Experience with chronic pain, balance, gait or functional rehabilitation
- Familiarity with VA Community Care, TriWest or authorization-based care
- Interest in whole-person healthcare, clinical outcomes or program development

This is not a traditional high-volume outpatient position. The DPT will initially focus on evaluations, re-evaluations, plan-of-care development and clinical oversight of an experienced Physical Therapist Assistant. Our administrative team will manage scheduling, authorizations, billing and routine payer communication.
The position will begin PRN while the provider completes credentialing and the physical therapy caseload develops. There is an opportunity for the role to grow into regular part-time or full-time work as referrals increase. New graduates who meet Arizona licensing requirements are welcome to apply!!!
Compensation
$100 per completed initial evaluation
$75 per completed re-evaluation
$65 per completed PT treatment visit when clinically necessary
$60 per hour for preapproved meetings, record review or administrative work
Compensation includes completion of the documentation associated with each encounter. Whenever possible, appointments will be grouped into efficient clinical blocks.
Provider participation is contingent upon successful credentialing and affiliation with Level Up Medical’s payer networks.
Credentialing may take approximately 90–120 days.
The selected provider cannot treat VA-authorized patients until enrollment is complete and an effective participation date has been confirmed.
