Primary Responsibilities:
- Diagnosis and treatment of acute, chronic, and long-term healthcare issues
- Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population certified in the Practice while administering quality patient care
- Maintains accurate medical records and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations
- Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications

Optum NY/NJ was formed in 2022 by bringing together Riverside Medical Group, CareMount Medical and ProHealth Care. The regional alignment combines resources and services across the care continuum - from preventative medicine to diagnostics to treatment and beyond across New York, New Jersey, and Southern Connecticut. As a Patient Centered Medical Home, Optum NY/NJ can provide patient-focused medical care to the entire family. Required Qualifications:
- Unrestricted licensure in the state of (NY) at time of employment
- Licensing: Registered Nurse with current unrestricted license to practice and use the title of Nurse Practitioner/Advanced Practice Nurse in NYS
- Certificate of completion from an accredited Nurse Practitioner or Physician Assistant program
- Unrestricted DEA license (or willing to obtain prior to start)
Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
