Mercyhealth, an award-winning fully integrated health care delivery organization based in Wisconsin and Illinois, is seeking an experienced Nurse Practitioner to join our Cancer Institute at Javon Bea Hospital - Riverside in Rockford, IL.

Essential Duties and Responsibilities

• Assess patient data and performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician
• Maintain timely consult schedule.
• Provide patient and family education to assist patient and family to understand role of palliative care in current health situation.
• Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.
• Record physical findings, and formulate plan and prognosis, based on patient condition.
• Coordinates care with Physician and other health professionals to prepare comprehensive patient care plan.
• Discusses health care plan and goals of individual patients for periodic review and evaluation by Physician.
• Refers patients to Physician for consultation or to specialized health resources for treatment.
• Assists in development of protocols.

Education and Experience

• Graduate of an accredited nursing program.
• Graduate of an accredited masters of nursing program.
• Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.

Certification and Licensure

• Hold a valid APRN & RN license in state of practice.
• Hold a valid DEA license in state of practice.
• CPR or BLS certification or other certifications as required
